Sarah is a Nurse Practitioner who has worked in nursing for over 17 years. She graduated with a Bachelor of Science degree in Nursing from Murray State University in 2009, and later graduated with her Master of Science in Nursing at Union University in Jackson, TN in 2012. She is board certified with the American Nurses Credentialing Center (ANCC). Her nursing experience included working in cardiac care, maternal/newborn care, urgent care, aesthetics, and regenerative medicine.
Sarah has lived in Murray, KY since 2005. She is married to a local area Murray City fireman, Mitchell Downey. Sarah and Mitchell have 4 children that keep them very busy–Reese, Logan, William and Easton.

Jacob is the behind-the-scenes magic that keeps the clinic running smoothly! With over 15 years of experience in healthcare, he’s the go-to person for just about everything. Over the years, he’s gained expertise in lab work, leadership roles, and pediatrics. Whether it’s solving a problem, answering a question, or supporting the team, Jacob is always ready with a calm presence and a can-do attitude.
Outside the clinic, Jacob loves spending time with his daughter, Ariana, who keeps life fun and full of heart. He’s also deeply passionate about supporting families with special needs. He dedicates time to helping families in the autism community find the resources and support their loved ones deserve. Whether he’s solving problems at the clinic or making a difference in the lives of others, Jacob’s mix of dedication, compassion, and leadership makes him an irreplaceable part of our team.
Meet Lexie! She is the first friendly face you see when you walk into Aligned Health. Her excellent customer service comes from her time working with Disney. She worked at both Walt Disney world in Florida and Disneyland in California. Lexie is a West Kentucky native who moved back home, met her husband William, and they now have a beautiful baby girl Dorothea. Their little family was made complete with their dog Ruby and their cats Pepa and Pumpkin. Lexie has been with us since October 2023, so she knows how to do a little of everything ! She can’t wait to greet you with a magical smile as you walk in our doors.
Meet Shayla! She came to us with several years of experience as a receptionist. She has made our front desk team complete with her beautiful smile and go-getter attitude. She always provides our patients with exceptional care. When she is not at work, she likes to spend time with her husband Brad and two children, Raigen and Hunter. She enjoys Jeeping in LBL and playing with her 3 fur babies. She may be one of the newest members of our team, but her knowledge and love for our patients shine through every day.
Kristin is Leslye’s medical assistant and clinic superstar. With over a decade of experience in the medical field, Kristin brings top-notch skills, compassion, and a can-do attitude to our team. She’s often the go to person in the clinic when someone needs a helping hand. Kristin’s friendly energy and team-first mindset makes her an essential part of our practice.
